An extraction is the removal of the crown (the top part of the tooth) and its root.  These are necessary for many different reasons, but usually because the tooth is not fixable or going to cause an infection.  Extractions are routinely done in our office using local anesthetic and if desired, nitrous oxide (laughing gas).   This procedure is not supposed to be painful and there are always ways to make the procedure more comfortable.  If someone would like to be asleep during the treatment, we can refer them to an oral surgeon who is trained in general anesthesia.  Treatment that is more difficult or involves certain medical problems may also be referred to an oral surgeon.
